Police begin investigation after toddler with 'strange rash and bloody sores' removed from foster mother in 'dire condition' days after two-year-old died at the same home

• Avalena Conway-Coxon and a 22-month-old girl were discovered in a three-bed subsidized apartment in Auburn, Massachusetts, Saturday 
• The 22-month-old is in 'dire condition' 
• Neighbors say the children had a strange rash characterized by open and bloody sores
• It was revealed that the foster mothers boyfriend was around the home during the day without approval from the state 
• Police records show officers had visited the home of the girls' foster mother 63 times between 2005 and 2014
